Our Community Impact
Since our founding in 2011, Live Health Foundation has remained accountable to the communities we serve through transparent reporting of program outcomes, financial stewardship, and measurable health improvements. This report reflects our cumulative impact and 2025 fiscal year performance.
Year in Review
Fiscal Year 2025 (July 1, 2024 – June 30, 2025) marked a significant expansion of our programming reach, with new partnerships enabling us to serve three additional zip codes in northeastern Mecklenburg County.
Community Wellness Screenings
Individual screenings completed across 16 quarterly events at 12 unique community locations. Of participants screened, 18% were identified with previously undiagnosed hypertension (blood pressure exceeding 140/90 mmHg) and connected to primary care providers for follow-up assessment. An additional 11% presented elevated fasting glucose levels consistent with prediabetes criteria.
Diabetes Prevention Workshop Series
Participants enrolled in CDC-recognized Diabetes Prevention Program cohorts during FY 2025, with a 76% completion rate (completing 9 or more of the 16 core sessions). Graduates achieved an average 5.4% reduction in body weight and a 29% improvement in fasting glucose levels. Twelve new Lifestyle Coaches were trained and credentialed through our partnership with the Association of Diabetes Care and Education Specialists.
Heart Health Initiative
Adults completed the 12-week cardiovascular wellness program. Participants demonstrated an average systolic blood pressure reduction of 9.1 mmHg and a 42% increase in self-reported weekly physical activity minutes. Volunteer cardiologists from Atrium Health and Novant Health contributed 320 pro-bono clinical hours across program sessions and educational seminars.
Youth Nutrition Education
Children and adolescents ages 8 – 17 participated in eight-week nutrition education tracks at 14 school and community center locations. Post-program surveys indicate a 35% increase in daily vegetable consumption and a 28% decrease in sugar-sweetened beverage intake among participants. Concurrent parent workshops served 190 caregivers.
Senior Fitness & Falls Prevention
Older adults aged 60 and above completed 10-week exercise and home safety program cycles. Follow-up assessments documented a 36% reduction in self-reported falls in the six months post-completion. Participants reported significant improvements in balance confidence (Timed Up and Go test improvement averaging 2.8 seconds) and independence in daily activities.

Where Your Dollars Go
Live Health Foundation maintains the highest standards of financial transparency and accountability. Our Board of Directors reviews audited financial statements quarterly, and we publish annual financial summaries for public review. We are committed to maximizing the direct community benefit of every dollar donated.
Screening supplies, curriculum materials, instructor compensation, venue costs, participant snacks, transportation vouchers, and program evaluation.
Staff salaries, office lease, insurance, technology, accounting, legal compliance, and Board governance support.
Grant writing, donor communications, annual gala coordination, corporate partnership development, and community awareness campaigns.
FY 2025 total revenue: $1,842,000. Audited financial statements are available upon request. Live Health Foundation has received a "Gold" transparency rating from GuideStar and maintains an A+ accountability rating from the Charlotte Foundation.
